But during this time, concerns about the baby’s health may arise. Decreased amniotic fluid is a condition that can be encountered during pregnancy, and it is very important to understand what it is, its causes, and what to do.

What is Amniotic Fluid?

Amniotic fluid is a clear, yellowish fluid that surrounds and protects the fetus during pregnancy. This fluid protects the fetus from external factors, provides it with room to move and offers the environment necessary for the development of the fetus. Also, some special properties of amniotic fluid are:

Protection from Impacts: Amniotic fluid protects the fetus from impacts, compressions and pressure on the mother’s abdominal wall.

Temperature Compensation: It provides a constant temperature for the fetus. This helps the fetus not to be affected by temperature fluctuations of the external environment.

Protection Against Infections: Amniotic fluid protects the fetus against potentially harmful bacteria.

Development and Movement: The amniotic fluid provides the space and environment for the fetus to move freely. These movements are very important for muscle and bone development.

Lung Development: The fetus swallows the amniotic fluid and then excretes it again. This process promotes the development of the lungs, which is necessary for postpartum breathing.

Amniotic fluid changes with the progression of pregnancy. At the beginning of pregnancy, amniotic fluid consists mainly of fluid from the mother, while towards the end of pregnancy, the urine of the fetus forms the main component of the amniotic fluid.

Abnormal amniotic fluid levels can sometimes be an indication of potential complications. For example, an excessive amount of amniotic fluid (polyhydramniosis) or an insufficient amount of amniotic fluid (oligohydramniosis) can pose a risk to the fetus. Therefore, doctor’s control and recommendations are very important during pregnancy.

Decrease in Amniotic Fluid

Why Does Amniotic Fluid Decrease?

A decrease in amniotic fluid is called oligohydramniosis. The causes of oligohydramniosis are varied, and sometimes no specific cause can be identified. But here are some common causes:

Prolongation of Gestational Period: 40 days of pregnancy From the week Then the amniotic fluid naturally begins to decrease.

Placenta Problems: The placenta provides blood and nutrients to the fetus. If the placenta is not working properly, the fetus cannot feed adequately, which can lead to a decrease in amniotic fluid.

Leak: Leakage of amniotic fluid as a result of rupture or leakage of the amniotic sac.

Some Medications: Some high blood pressure medications, in particular, can cause a decrease in amniotic fluid.

Kidney Problems of the Fetus: If there is a problem with the kidneys or urinary tract of the fetus, it can cause a decrease in the production of amniotic fluid. That’s because the fetus’s urine is an important component of the amniotic fluid later in pregnancy.

Chronic Hypertension: Chronic high blood pressure in the mother.

Dehydration: Insufficient fluid intake or excessive fluid loss of the expectant mother.

Infections: Intrauterine infections can cause a decrease in amniotic fluid.

Oligohydramniosis is usually diagnosed by ultrasound. If the amniotic fluid is low, the doctor recommends frequent check-ups and follow-ups. Depending on the situation, treatments may be applied to increase fluid levels or a decision may be made to deliver prematurely.

A decrease in amniotic fluid can bring some risks to the fetus. Therefore, when such a situation is encountered, doctor’s advice and guidance are essential.

How to Understand the Decrease in Amniotic Fluid? What are the symptoms?

The suspicion of reduced amniotic fluid (oligohydramniosis) is usually determined by doctors during routine prenatal examinations or as a result of certain symptoms. However, special medical tools, such as ultrasound, are needed to definitively detect this condition. Some possible symptoms of oligohydramniosis include:

Low Abdominal Volume: Later in pregnancy, the abdomen appears smaller than expected.

Decreased fetal movement: Feeling a decrease in baby movements. However, this does not mean that amniotic fluid is reduced. Some babies are less active, or the way the mother feels baby movements may change.

Leak: Leakage of amniotic fluid, the feeling that water is coming out. This means that the amniotic sac has ruptured and may require immediate medical attention.

Compression Symptoms: During follow-ups with a fetal monitor, there may be drops in infant heartbeat when the baby moves or during contractions such as Braxton Hicks contractions.

If you notice any or more of these symptoms, you should contact your doctor. However, the definitive diagnosis of oligohydramniosis can be made by ultrasound. During an ultrasound, the doctor measures the volume of amniotic fluid, which is expressed in a value known as the amnionic fluid index (AFI). When the AFI value is less than 5 cm, oligohydramniosis is usually diagnosed.

If an oligohydramniosis is suspected, the doctor should take immediate action. A decrease in amniotic fluid can bring some risks for both mother and baby.

Decrease in Amniotic Fluid

What Is Good For Reducing Amniotic Fluid?

The treatment or approaches that can be applied in case of decreased amniotic fluid (oligohydramniosis) may vary depending on the situation, duration of pregnancy, causes and general health status of the mother. However, some common approaches to the management of oligohydramniosis include:

Increasing Fluid Intake: The expectant mother may be advised to drink plenty of fluids. Adequate hydration can increase amniotic fluid levels.

Amnioeplesion (Amniotic Fluid Supplement): This is a procedure that is performed especially when the amniotic fluid is very low and close to birth. A sterile fluid is injected into the amniotic sac through a thin needle used for amniocentesis.

Review of Medications: If certain medications are being used that cause a decrease in amniotic fluid, the doctor may review or change the use of these medications.

Bed Rest: In some cases, it may be recommended to take the mother to bed rest. This can help increase fluid.

Frequent Follow-up: In the case of oligohydramniosis, the doctor may want to regularly check the condition of the fetus and the level of amniotic fluid. This is done by ultrasound.

Preterm Birth: If the condition is serious and poses a risk to the baby, the doctor may decide to give birth prematurely.

Decisions about what is good for the reduction of amniotic fluid should be made with the recommendations and guidance of the doctor. If there is a suspicion of a decrease in amniotic fluid or if this condition has been diagnosed, it is essential to act in accordance with the doctor’s recommendations and guidelines. Each situation and patient is unique, so it’s best to follow individual medical advice rather than general recommendations.

How Much Amniotic Fluid Should Be? How Is It Measured?

The amount of amniotic fluid in a healthy pregnancy varies depending on the duration of the pregnancy. The most common method used to measure the amount of amniotic fluid is the amnionic fluid index (AFI) measurement. AFI is measured by ultrasound as the sum of the depth of the fluid pocket in four separate uterine regions.

In general, AFI values in a healthy pregnancy are as follows:

Between Weeks 24-28: It can go up to about 25 cm, which is the peak value.

As the deadline approaches: The AFI value is usually reduced and can usually have a value between 8-12 cm at the time of birth.

An AFI value is:

If it is less than 5 cm, this is usually considered oligohydramniosis (decreased amniotic fluid).

If it is over 24-25 cm, this is usually considered polyhydramniosis (excess amniotic fluid).

However, these values are used as a general guideline and may differ for each individual. Also, it is important to note that there may be slight variations between different ultrasound machines and techniques.

How Is Amniotic Fluid Reduction Treated?

The approaches that can be applied in the treatment of decreased amniotic fluid (oligohydramniosis) may vary depending on the severity of the condition, the duration of pregnancy, the causes and the general health status of the mother. Here are some common treatment methods for the management of oligohydramniosis:

Increasing Fluid Intake: The mother’s drinking of plenty of fluids can increase amniotic fluid levels in some cases.

Amnioeplesion (Amniotic Fluid Supplement): This procedure, in which a sterile fluid is injected into the amniotic sac, is often performed in the case of oligohydramniosis, especially during childbirth. This can help reduce the pressure on the fetus.

Medications: Certain medications can promote an increase in amniotic fluid. For example, oral waters (osmotic diuretics), which can increase amniotic fluid levels by increasing fluid retention in the mother, are sometimes used.

Review of Medications: If certain medications are being used that cause a decrease in amniotic fluid (for example, some high blood pressure medications), the doctor may review or change the use of these medications.

Bed Rest: In some cases, it may be recommended to take the mother to bed rest. This can help increase fluid.

Regular Follow-up: In the case of oligohydramniosis, the doctor may want to regularly check the condition of the fetus and the level of amniotic fluid. This is usually done with ultrasound.

Preterm Birth: If the condition is serious and poses a risk to the baby, the doctor may decide to give birth prematurely.

When determining a specific approach to the treatment of oligohydramniosis, it is essential to determine the cause and assess the overall health of the baby and mother.

When Does Amniotic Fluid Decrease Occur?

Decreased amniotic fluid (oligohydramniosis) can occur at any time of pregnancy, but it is more common in some periods:

First Trimester and Second Trimester Onset: Although the decrease in amniotic fluid is rarely seen during these periods, if it does occur, it can often be a sign of serious fetal abnormalities, chromosomal disorders, or placental problems. The prognosis of oligohydramniosis during this period is usually poor and can lead to serious consequences.

Late Second Trimester and Third Trimester: Oligohydramniosis is more common during these periods. Among the common causes of oligohydramniosis during this period are:

Premature aging of the placenta

Maternal conditions such as hypertension or preeclampsia

Abnormal function of the baby’s kidneys or urinary tract

Leakage or rupture of the amniotic sac

Side effects of certain medications (e.g. some high blood pressure medications)

It is present in these periods.

When Approaching or Passing a Deadline: After the 40th week of pregnancy, a natural decrease in amniotic fluid occurs. However, excessive reduction during this period (especially due to reduced placental function) can be risky for the fetus.

Frequently Asked Questions

Is a decrease in amniotic fluid always a serious problem?

In some mild cases, this condition may not pose a serious threat.
